Completing the CSMC Patient Registration Form
We are committed to providing our patients with high quality health care. In order to achieve this it is essential that we have an up to date and accurate health record.
We require ALL NEW patients to complete and submit the Patient Registration Form prior to your booked appointment. Forms are submitted via our secure on-line form link.
If you do NOT complete/submit the form, you will need to arrive at least 20 minutes prior to your scheduled appointment time so that you can complete the required registration paperwork.
Returning Patients: It’s been a while?
We would also ask that if you have not been to see your GP within the past 12 months you may also take the time to complete the form; this will allow us to ensure we have your up to date details on our system.
These forms will auto-fill your data when using the same browser on the same computer for up to a day. This allows you extra time to complete the form. Patients are encouraged to print a copy of their information before submitting these forms.
Improving our Communication with our Patients.
The practice is increasingly seeking to communicate with our patients via secure electronic means. This would include email / SMS health reminders, on-line appointment bookings, SMS appointment reminders and in the future video consultations and patient uploading basic health information. To do this we need to ensure we have your email address, mobile phone number and of course your consent.
If you haven’t been to see your GP for some time or haven’t updated you contact / personal details for some time you can provide your current information by completing and submitting the CSMC Registration Form.
We believe in providing our patients with appropriate preventative health information, recall / reminders and other health related information that will support your health and well-being. As a patient of CSMC you have the option to be included in our preventative health management program; you have the right to opt-out and should you wish to do so please advise one of our team or your GP.

Patient Request for Access/ Release of Personal Health Information
Wherever possible, Doctors are available between 12 noon – 1 pm to receive telephone calls. Our Practice Nurses are also available to receive calls. However, consultations cannot take place over the telephone so you may require a follow-up consultation.
Our reception staff are available to make bookings and to assist you as required.
